Becoming an Anonymous Living Donor:
The Process

 

Step 1: Read through the materials provided to you, and review the following documents:

  • Anonymous Living Donor Program Information
  • Frequently Asked Questions
  • Health Screening Questionnaire
  • Authorization to Release Information

Step 2: Contact the Anonymous Living Donor Program by e-mail (baynesg@ohsu.edu) or by phone (503-494-7856) to ask any questions and express your interest in donating a kidney.

Step 3: Complete the Health Screening Questionnaire and the Authorization to Use and Disclose Protected Health Information and return them by mail to the Anonymous Living Donor Program at the following address:

After we receive the questionnaire, it will be reviewed to ensure you meet health criteria. We will then arrange an appointment for you to meet with a donor coordinator to discuss the process of donation in more detail and to answer any questions you might have.

Step 4: Once you have made the decision to donate a kidney, you will be referred to a donor coordinator at one of the two area transplant centers — either Oregon Health & Science University Hospital or Legacy Good Samaritan Hospital — for a more thorough health evaluation.

Step 5: The donor coordinator will walk you through the medical and psychological evaluations that will be completed at that transplant center. Once those tests are completed, you will be matched with a recipient — anonymously. At that point your donor surgery and the recipient’s transplant surgery will be scheduled at a time appropriate to both you and the recipient.
